Passenger Sidebar Form Evolution¶

What this is¶
A tight, vertically compact form variant designed to live in the sidebar of passenger-injury pages. The regular intake form was too tall for sidebar placement; this variant condenses the same fields with reduced vertical rhythm and a simplified header.
Iteration timeline¶
- April 9 — Placement preview (where it would sit on the page)
- April 10 — Tight title preview (compressed header)
- April 10 — Tight form preview (compressed body)
- April 10 — Live publish
Design constraints¶
- Must fit within the Kadence sidebar column without breaking layout
- Same fields as the main intake form
- Consistent branding with the SA Template System
- Compatible with the Custom Intake Rollout backend (pushes to Litify, not Gravity Forms)
